As we conclude 2024, we take a moment to acknowledge the remarkable journey we experienced together through our Type 2 Diabetes Group Visit Program. Under the proficient guidance of our Nurse Practitioner, Gardner Mack, participants acquired essential knowledge in diabetes management while fostering a supportive peer community.
