Investigators from the University of Sydney’s Center for Rural Health have identified a potential remedy. Their investigation, published in JAMA Network Open, demonstrated that incorporating lifestyle support in back pain treatment could minimize disability and improve life quality.

The randomized controlled study encompassed 346 subjects from various regions of Australia, all of whom experienced chronic lower back pain and possessed at least one lifestyle risk factor, including obesity, unhealthy diet, lack of physical activity, or smoking. Participants were randomly divided into two groups: the “Healthy Lifestyle Program (HeLP)” or standard physiotherapy following guidelines to manage their lower back discomfort.

The HeLP group received assistance from physiotherapists, nutritionists, and telehealth coaches who guided them in identifying which lifestyle practices might be affecting their back pain, such as weight management, inactivity, inadequate diet, poor sleep quality, smoking habits, or excessive alcohol consumption. They were subsequently given evidence-based recommendations over a six-month period to help tackle these lifestyle challenges.

This method exhibited several advantages compared to standard care, especially in terms of reducing disability, as participants showed an average score of 1.3 points lower on the Roland Morris Disability Questionnaire (where higher scores denote increased disability). HeLP participants also lost an average of 1.6 kg more than those in the control group.

Chief Investigator Associate Professor Chris Williams pointed out that the results challenge conventional perceptions of back pain management. “Addressing back pain requires a broader perspective than just focusing on the spine. Our bodies behave more like ecosystems, where multiple factors interact to influence our functioning and feelings. Back pain follows this same principle.”

“An increasing number of studies have indicated that issues like herniated discs and joint degeneration are seldom the main reasons for prolonged back pain,” stated Associate Professor Williams. “The issue lies in the fact that most individuals are not informed of this, and even fewer are offered support to concentrate on the elements that impact long-term pain and disability. A plethora of patients are referred for surgeries or prescribed medications that fail to provide relief—and may even worsen their condition.”

The study emphasizes the necessity for a fundamental change in back pain treatment, steering away from medication and surgical interventions towards addressing the lifestyle elements that can significantly influence outcomes.

Lead author and Senior Research Officer at the University of Sydney, Dr. Emma Mudd accentuated the practical implications the program could generate. “Numerous individuals struggling with prolonged back pain inform us that they feel neglected, frequently being referred to high-cost and ineffective solutions without receiving recommended strategies that foster self-management,” Dr. Mudd shared.

“By shifting the emphasis to incorporate lifestyle modifications and offering straightforward, supportive interventions, patients gain a sense of empowerment over their pain management. This strategy not only alleviates their symptoms but also enhances their overall life quality.”

The researchers believe that lifestyle-inclusive care, such as the HeLP initiative, may benefit patients beyond merely managing pain, potentially lowering the risks of other chronic illnesses. Nevertheless, Dr. Mudd remarked that worldwide guidelines have yet to fully adopt this methodology:

“Our findings could influence future revisions of back pain treatment guidelines. Patients appreciated the comprehensive support offered, and the results speak volumes.”

“Practitioners handling back pain should contemplate how they incorporate lifestyle support into their everyday practice. There doesn’t seem to be a single correct method to accomplish this, as long as the patient feels acknowledged and involved in the decision-making process,” remarked Associate Professor Williams.
