Communities experiencing the greatest burden of chronic disease often have the least access to clinicians who share their cultural backgrounds and can deliver care that is both evidence-based and culturally responsive. The HEAL Initiative Scholarship helps mitigate this barrier to ensure communities of color, rural populations, and other historically under-resourced groups have an expanded pool of providers who understand their lived experiences. While these groups continue to face disproportionately high rates of diabetes, hypertension, cardiovascular disease, obesity, and other chronic conditions, ACLM is committed to strategies that address these disparities and ensure that all patients have access to clinicians who understand their unique experiences and challenges.

A cornerstone of the initiative’s success has been ACLM’s commitment to building strategic partnerships with organizations that are deeply embedded in underserved communities. Through collaborations with organizations such as the National Association of Community Health Centers (NACHC), National Medical Association (NMA), and the National Association of Hispanic Nurses (NAHN), ACLM promotes HEAL scholarship opportunities directly to clinicians practicing on the front lines of care in communities disproportionately affected by chronic disease. These partnerships help ensure that the program reaches healthcare professionals already serving high-need patient populations and creates pathways for them to implement lifestyle medicine interventions.
